Incident: An unplanned, undesired event that caused or could have caused injury, damage, or loss (see Accident, Near Miss).

  • Accident: An incident that results in injury, damage, or loss. 
  • Near Miss: An incident that, under slightly different circumstances, could have resulted in injury, damage, or loss

Investigations seek facts in order to improve workplace health & safety, not to find fault or lay blame.

All incidents are investigated to 

  • Determine what actually happened.
  • Identify the causes (i.e., unsafe conditions, acts, or procedures).
  • Put controls in place to prevent future incidents.

This is a fictional example of a completed incident investigation form.  It can be used as a guide for filling out investigation forms.

Used to report safety, environmental, property damage, and ergonomic concerns.  Allows unsafe conditions to be corrected before an incident takes place.

Not to be confused with Incident Reports, which report events rather than conditions.

As soon as possible after an NTPC or contractor Medium or High RPH incident, work shall stop and the manager, Project Manager, or Worksite Monitor shall hold a safety meeting with all workers involved, including contractors.  The safety stand-down meeting is an opportunity to:

  • Show commitment to safety.
  • Understand what happened and why.
  • Ensure the workplace is safe before workers return to work.
  • Communicate safety expectations going forward.

Details of the meeting shall be documented on Form 10.9: Post Incident Safety Stand-Down, which includes discussion points, an attendance sheet, and an action plan.  This meeting will also help in the preparation of thorough incident and investigation reports.
