- Details of the emergency (i.e., level, date, time, location, description)
- Reasons for declaration
- Parties notified
- Resources deployed
- Lessons learned
- Debrief

Plan of action to prevent incidents on minor projects. Details health & safety systems/requirements for project. Reviewed with all workers and kept onsite. Required for all projects requiring contractors that do not qualify as major projects. General guidelines to use:
- Project work costs are less than $250,000 (excluding material costs);
- Only one contractor will be on site;
- Project duration is expected to be fewer than 15 days;
- Mainly low to medium risk activities; and/or
- As otherwise directed by the Health & Safety Department.
